Please Read and Accept the Following agreement

If selected, I agree to abide by all rules and regulations of the North Metro Ballers, and the JAM/Gold Crown leagues. I further agree to not mislead the Ballers Executive Board or misrepresent the Ballers.
If selected, I understand that I will be required to provide a driver's license and personal/business references for any required background checks.
If selected, I understand that I will be required to read and abide by the Ballers Code of Conduct and any other Codes required by JAM.
I hereby certify that I have never been convicted of a felony, including alcohol related driving felonies.
I hereby certify that I have never been convicted of, or charged with any form of child abuse.
I hereby certify that I have never been removed as a coach, teacher, or any other type of instructor position due to misconduct.
I hereby assume all risks and hazards incidental to participation including transportation to and from activities and I understand that the Ballers do not carry accident insurance.
If selected, I agree to pay all costs of any lost equipment issued to me by the Ballers.
The Ballers reserve the right to modify this agreement at any time. The Coach is expected to remain informed about any policy changes and is expected to abide by them.
